WILLIAMSBURG SCHOOLS’ SCHOOL HEALTH PROGRAM

The Massachusetts Department of Public Health (MDPH) is the state governing agency that oversees school health programs. Many of the forms we use and health related information that we collect is according the MDPH law. Upon entrance, and annually thereafter, we collect a lot of health-related information and require parents and/or legal guardians to complete several forms.

FORMS

Emergency Information Card: This MUST be completed at the beginning of every school year and upon entering school for new students.

Planning For Kindergarten: This information sheet helps parents plan accordingly in regards to health guidelines that need to be followed prior to kindergarten entrance.

Planning for Preschool Entry - This information sheet helps parents plan accordingly in regards to health guidelines that need to be followed prior to preschool entrance.

Mass General Laws, Chapter 111, s.219 requires us to provide parents of children attending preschool and kindergarten information regarding meningococcal disease. Please see the MDPH handout for more information.

Massachusetts School Health Record (physical examination): This for is required for all preschool and kindergarten students as well as students going into 4th grade. Students will also need to have a physical prior to entering grade 7 at Hampshire Regional.

Attendance Guidelines: Do you know when you can safely send your child back to school? This information will help guide parents/guardians about whether or not their child should be sent to school if they have been sick.

MDPH Physical Exam Form - Required for all new students, pre-k entry, kindergarten entry, grade 4 and for students entering grade 7 at HRHS

Medication Policy: This is an information sheet for all students that is provided at the beginning of the year or to new students entering Williamsburg Schools. This sheet will help guide parents/guardians about the use and administration of medication during school hour. If medication during school hours will be necessary you will need to complete the two forms below before medication will be administered to your child by the school nurse.

Medication Order Form (Physician): In order for children to receive medication during school hours your child’s physician/health care provider must complete a written order.

Parent/Guardian Consent For Medication: In order for children to receive medication during school hours a parent/guardian consent must be on file along with the health care providers’ written order.

Fluoride Administration Consent: If your child is in grades 1 through 6 he/she may participate in our once a week fluoride mouthrinse program. In order for your child to participate a parent/guardian MUST complete the fluoride consent form and return it to the school nurse.
